Philadelphia School District's Office of Specialized Services offers the following transition services:
Transition Services can be thought of as a bridge between school
programs and the opportunities of adult life. Federal law (IDEA 2004)
defines transition services as "a coordinated set of activities for a
child with a disability that is designed within a results-oriented
process, that is focused on improving the academic and functional
achievement of the child to facilitate the child's movement from school
to post-school activities."
Post school activities include:
The coordinated set of activities is based upon a child's needs, and
takes into account a child's strengths, preferences, and interests.
