Philadelphia School District's Office of Specialized Services offers the following transition services:

Transition Services can be thought of as a bridge between school programs and the opportunities of adult life. Federal law (IDEA 2004) defines transition services as "a coordinated set of activities for a child with a disability that is designed within a results-oriented process, that is focused on improving the academic and functional achievement of the child to facilitate the child's movement from school to post-school activities."

Post school activities include:

  • Post-secondary education
  • Vocational education
  • Integrated employment (including supported employment)
  • Continuing education
  • Adult education
  • Independent living
  • Community participation


The coordinated set of activities is based upon a child's needs, and takes into account a child's strengths, preferences, and interests. School related activities include:

  • Instruction
  • Related services
  • Community experiences
  • Development of employment and other post-school adult living objectives
  • Acquisition of daily living skills, when appropriate
  • Functional vocational evaluation, when appropriate
