The transition from school to adult life is a winding road that many young people with developmental disabilities and their families find difficult to navigate.

TransitionMap's mission is to provide a roadmap of resources and services that will enhance the lives of young people ages 14 to 21 on their journey to adulthood.

TransitionMap is a regional partnership sponsored by KenCrest for the community of youth, their families and the professionals who support them.
